Hello.
1. You need connect phone direct to mother board. Dont use any hub or box for connections.
2. You need download and install latest LG driver (
http://tool.lime.gdms.lge.com/dn/downloader.dev?fileKey=UW00120120425
)
3. After this put phone in download mode and repeat unlock operation.
